Last week it was Racist Bannon, this week Trump himself and various fellow travelers pretending to dare and double dare the Democrats...fuck that.
Easiest answer: if there are impeachable offenses, impeach (um, there are -- duh). Who decides? That's a little trickier, given that impeachment is political. However, you could base the political act of impeachment on, I dunno, the Mueller report, or even on Trump's own words.
And if this were a Democratic president, impeachment would be the least of their worries at this point. No way would Obama have made it this far, and I doubt Bill Clinton would have either. Hillary Clinton? Don't make me laugh: the GOP would impeach her for breathing.
